Changes around the mouth can affect the way the whole face appears. Vertical lip lines may become visible at rest, the corners of the mouth can begin to turn down and the skin around the mouth and chin may lose some of its smoothness and resilience.
These changes rarely have one cause. Repeated facial movement, declining collagen and cumulative sun exposure can all contribute. Dehydration may make superficial creasing more apparent, while the balance of the muscles around the mouth can influence lip position, chin texture and the way the smile appears.
At Noaem Skin, treatment may include conservative botulinun toxin, injectable skin treatments, microneedling or MOXI laser. Skincare may also form part of the plan. During consultation Dr Chris will determine whether one treatment or a combination is appropriate following assessment.
Treatment options
Repeated movement of the muscles around the mouth can contribute to vertical upper-lip lines, a downturned mouth, chin dimpling or the appearance of a gummy smile.
Very small amounts of Botulinum toxin, commonly know as Botox, can be used to alter the activity of selected muscles. This may soften dynamic lip lines or change the balance of movement around the mouth without removing natural expression.
Redensity 1 is used to improve hydration and superficial skin quality. It may be helpful where the skin around the mouth appears finely creased, depleted or less able to retain moisture.
Injected polynucleotides work differently. They support repair processes within the skin and may be recommended where the tissue has become thinner or less resilient. Improvement develops gradually across a course rather than creating an immediate change.
Redensity 1 and polynucleotide intjections around the mouth may be performed by Dr Chris or Iki Rhode.
Microneedling creates controlled microchannels that stimulate the skin’s repair response and support new collagen production. At Noaem Skin, it is provided through combined Dermapen 4 TriLuxe and PolyLuxe treatments.
Dermapen 4 TriLuxe combines microneedling with mesotherapy solutions and the Dermapen ÜBER PRO Peel. Dermapen 4 PolyLuxe combines microneedling with topical polynucleotides. The appropriate option depends on whether surface refinement or broader tissues quality is the greater priority.
A corrective course is generally required because collagen remodelling develops progressively rather that after a single session. Both treatments are performed by Iki Rhode.
MOXI is a non-abaltive fractional laser that may be used where fine lines are accompanied by roughness, uneven pigmentation aor wider sun damage around the mouth. MOXI addresses changes within the skin rather than muscular movement. Where both factors contribute, Dr Chris may recommend sequencing it with another treatment.
A retinoid may support cell renewal and collagen production, while daily broad-spectrum SPF helps prevent further ultraviolet damage. Antioxidant skincare may also form part of the recommended routine.
What causes changes around the mouth?
Vertical lines above the upper lip develop through repeated orbicularis oris movement and declining dermal collagen. Sun exposure and smoking can accelerate them, although they also occur in people who have never smoked. Lines that deepen on pursing have a stronger dynamic component, while those visible at rest usually involve more established change within the skin.
The depressor anguli oris muscles can pull the corners of the mouth downwards, making someone look unhappy or tired when they do feel that way. Conservative Botulinum toxin treatment may be appropriate where muscular activity is a significant factor.
A gummy smile occurs when the upper lip rises sufficiently during smiling to reveal more gum than the patient would like. Botulinum toxin may reduce this elevation in suitable patients, although dental and facial anatomy also need to be assessed.
A puckered or dimpled chin can result from the overactivity of the mentalis muscle. Dr Chris will assess whether relaxing it could create a smoother appearance without affecting normal movement.
A lip flip uses small amounts of botulinum toxin to relax part of the muscle around the upper lip, allowing a little more of the lip to show. It changes the lip position rather than adding volume.
Loss of collagen and hydration can make the skin around the mouth less resilient. Sun exposure may add pigmentation, roughness and fine surface lines. These changes may respond better to treatment directed at the skin than the muscle activity alone.
During perimenopause and menopause, declining oestrogen also affects collagen production and the skin’s ability to retain moisture. Fine creasing may become more noticeable where hormonal change compounds sun damage and repeated movement.

Dr Chris will examine the skin and assess how the mouth and lower face move during expression. HE will distinguish changes cause mainly by muscular activity from those related to collagen loss, dehydration or photodamage, then determine whether one treatment or a combination is appropriate.

Botulinum toxin is usually repeated every three to four months to maintain the effect. Injectable skin treatments, microneedling and MOXI are generally recommended as corrective courses. The number and sequence will be confirmed following consultation.
Injections can cause temporary redness, swelling or bruising. Microneedling usually leaves the skin flushed. MOXI produces redness followed by a dry or lightly bronzed texture that sheds over several days.
Yes. Where both dynamic and static lines are present, treatment for repeated muscular folding may be combined with a course focused on skin quality. Dr Chris will determine the sequence.
